HRGO are recruiting for a Carers Respite Coordinator. The post holder is responsible for providing a pro-active service to Carers, so as to support the delivery of respite services through health funded initiatives. Core Duties * Work with Carers and support teams to identify and support respite care needs * Establish and maintain positive relationships with statutory and non-statutory agencies and care providers to facilitate coordinated respite care packages of support to Carers and those care for * Arrange packages of support for Carers upon their Cared for persons discharge from hospital * Provide day to day administrative functions to support the respite care coordination service * Provide appropriate levels of information to care providers supporting compliance with their legislative responsibilities * Undertake initial assessment to ensure referrals meet the eligibility criteria and are compliant with contractual obligations, the Care Act and Safeguarding * Provide a sensitive and informed response to carers Essential * Experience of working with people with care needs and/or Carers * Experience of dealing with confidential and sensitive matters * Experience of working to policies and procedures * Knowledge /understanding of domiciliary care provision * Awareness of the needs of Carers and those cared for * Experience of using database and/or information systems This is a full-time position, 37.5 hours per week, and is an excellent opportunity to work with a successful and well-established company.
