HRGO are recruiting for a Temporary Hub Co-ordinator. The post holder will provide appropriate up to date information, advice, support and assessment to Carers to support them in their caring role and undertake non-statutory assessment from the Carers Hub. You must have experience of working with people with care needs and/or Carers.
Core Duties
Provision of a telephone triage, assessment and support service to Carers from our Carers Hub
Respond appropriately to calls received in the Carers Hub, directing the caller to the most appropriate team member
Manage incoming referrals, ensuring procedures are followed and information systems are updated
Provision of information to Carers and professionals over the telephone, by e-mail and during visits to the Carer Centre
Provision of emotional support (not counselling) to Carers
Desirable
Some understanding of key issues and developments across Health & Social Care
Knowledge of Family Social Care processes
Knowledge of NHS & Integrated Discharge Team processes within hospitals
Knowledge of welfare benefits and services available to carers
Awareness of how care funding works and applies to Carers and those with care needs
Awareness of respite care provision and primary
This is a full-time temporary position, 37.5 hours per week, and is an excellent opportunity to work with a successful and well-established company.
