Due to continuous growth and development of the organisation we are now looking for a Care Coordinator (Locality Manager) You will be responsible for ensuring that services within your locality reflect the requests of disabled individuals. This includes ensuring that your locality is fully resourced in terms of staff that services are being delivered in line with agreed person centred support plans and that individual outcomes are being achieved. You will work collaboratively with other Locality Managers, to line manage support staff based in your locality and this will include responsibility for recruitment, performance management as well as ensuring that cover for sickness and annual leave is in place. To be successful you will need to have proven experience in: * health and social care, working with elderly people who are supported within their own homes * 1 years management experience * creating and adhering to policies and procedures, ensuring that the Company is always working within legislation guidelines * maintaining relations with local authorities, family members and others who are involved with the Service * ensuring that the service is adhering to care requirements, for example hours to be provided and monitoring care packages are within price plans * creating, updating and reviewing person centered care plans We pride ourselves with our hands on and personal approach, to Service Users and employees alike, so we are looking for someone who strives for perfection whilst keeping it real! Ideally you will have a NVQ level 3 or be willing to work towards but you must hold a full clean driving licence.
